Introducing a Caregiver Support Program for Formal and Informal Caregivers
Family caregivers remain an important part of the support network for many residents in long-term care environments. When caregivers are informed, organized, and able to communicate effectively, it can strengthen the overall care experience for both residents and the care team.

To support this need, Key Health Care developed caregiver support programs designed to help families coordinate care, share information, and remain constructively engaged in the care process.
This proposal outlines how the Care That Shares™ caregiver education initiative, along with the Key Caregivers and CompassionateCare ContinuumRx programs, may support caregiver coordination while reinforcing communication practices that align with federal long-term care expectations.
